Please note: A complete list of of each community can be found here.To apply during the opening period, pick up an application at the JHA office, located at 501 Chestnut St, Johnstown, PA 15906, from 8:00 am until 4:00 pm ET, Monday-Friday.Once the application has been completed, it can be hand delivered to the address above, or mailed to P.O.
