Stanton St., Roseburg, Oregon 97470, Monday-Thursday between the hours of 9:00 am and 4:00 pm, and Fridays from 9:00 am to 12:00 pm.Once the application has been completed, it can be mailed or hand delivered to the address listed above.No preferences are noted.
