Visit the AHA to pick up an application, located at 200 Oxford St, North, Auburn, MA 01501, between the hours of 9:00 am and 3:00 pm Monday-Friday.2. Request an application be mailed to you by calling 508-832-3852 between the hours of 9:00 am and 3:00 pm, Monday-Friday.3.
